PDF Form Creator

Create a fillable PDF in minutes - add text boxes, checkboxes, radio buttons, dropdowns, date and signature fields to any PDF, or start from a blank page. The fields are standard PDF form fields, so people can fill them in Acrobat, Chrome or Sign PDF. Everything happens in your browser.

How to Create a Fillable PDF

  1. Open your PDF. Drop the PDF into the box, or start from a blank A4 or Letter page.
  2. Add form fields. Pick a field type - text, multi-line, checkbox, radio, dropdown, date or signature - and click the page where it goes.
  3. Set the details. Drag and resize each field, then give it a name, default value, options, and mark it required or read-only.
  4. Download the fillable PDF. Press Create fillable PDF. The form works in Acrobat, Chrome and every common PDF reader.

No PDF handy? Try a sample form and press Find blank lines to watch the fields appear by themselves.

Real Form Fields, Made Privately

The tool writes standard PDF form fields, the kind Adobe Acrobat makes. Anyone can fill them in a browser or a free PDF reader, and the answers are saved with the file.

Your PDF never leaves your device - it is opened and saved right in this tab. When the form comes back filled in, Sign PDF can add a signature, and PDF to Word turns it into an editable document.

Tips for Better Forms

  • Name fields clearly, like first_name or start_date - the names show up when answers are exported.
  • Use tooltips to explain what goes in a box. Screen readers read them aloud.
  • Same box on every page? Select it and press All pages.
  • Scanned paper form? Run OCR PDF first so Find blank lines can see the text.
